I've heard that some of them have that option. All of them are not that way though, I would think. They offer massages, right?

They are in the same class as massage parlors and barber shops.  Some are just a front for a brothel, some are clean, but most can go either way depending on your request.

In general, I would have to say no, they are not at all like brothels, and in fact, some are even segregated by sex, with young kids going with their mamas.  And some are just everyone together, like those called "public bath houses", open for passerbyes to look at whomever is there.

and just like there could be a rotten apple in a barrel of good apples, I am sure that there are some that do have "special' services for a fee, and special rooms to perform those services.  But the ciustom of public baths is still too deep in Chinese culture, especially in country areas where homes do not normally have a so called "bath", but some sort of "out house" nearby, those requiring a place to take a bath besides a river or nearby lake.  Just remember, Chinese idea of "nudity" is not at all like ours, and they do act and behave on the idea that the rest of the people around them really do not Look at what they are doing.
