A friend asked me this and I said I'm not 100% sure, but I think once the person exits from China, even when going to Hong Kong, they can't re-enter China on that same visa. Is that true?

That is true, if you are in China on a Single Entry visa, and you go to Hong Kong (or Macau for that matter) you will not get back in.
If you do go to Hong Kong, you can apply for a new visa while you are there. Search for threads on the topic

Went through some posts, and all mention about going to Hong Kong to apply for a new visa. But to apply for one means you have to have your country's embassy there though right? We don't have a consulate or embassy in Hong Kong, so does she can't apply for a visa there, I'm guessing.

No, visa for China, comes from a Chinese embassy or consulate. There are visa agents too. Effectively what the agents do is, take your passport, send someone on the subway to the border to China, hand over the passport to the PSB, and then bring it back. I've only done it once some years ago, got a 2 entry tourist visa in Hong Kong. In most cases you have to wait at least a day, so you need to look into accommodation too.
Technically it is not an Embassy, it is called The Commissioner's Office. It is located at
7th Floor, Lower Block, China Resources Building,
No.26, Harbor Road, Wanchai, Hong Kong SAR
Office Hours
Monday to Friday (except Hong Kong public holidays,)
9:00-12:00 and 14:00-17:00
And they do issue visas there, but they are selective according to your country of origin, and visas issued are one or two entry, maybe up to 6 months validity, 30 days stay mostly. But I have seen some rare cases where better visas were issued.
Not really, depend on where did the single entry visa issued.
if the single entry visa was issued by some embassy outside of mainland China, you through the border entered China, then go through border to HK means your 1 entry run out, even your current visa still have some duration, but you can not come back with it any more;
if the single entry visa was issued by some PSB office in mainland of China, and you never use this single entry go out of China after you obtained this visa, then go to HK and possible to back China again with the visa still in the valid duration.
