I have been entering and exiting China since 1997, and I was never asked for it when I left.  But I do carry it with me just in case they do ask for it.

But at every visit to PSB, for whatever reason, that is the first thing they ask me to give to them

I do save all of them as a way to prove of my stay in China.

no...no need..

But you should keep it....

My passport got damaged here and at that time it was just that form that help me to get the Visa again (without much running around)
