Yes, each time you renew your RP you need to register at PSB. Bring along your passport, a photo and a copy of your rental contract. They will need all these documents to ensure you are a bona vide person working there.

I renewed my Residential permit twice but never went to police station to register after each renewal. Last week, I went to PSB for extending my wife's visa as she came here on "L visa." That time I was told that I need to register to local police again because I changed my passport, i.e. new passport no. So based on PSB, as I was told, I need to register only if I change my passport or change address or change job.

First time when I came to China, I was told at police station that I need to inform police station about my every exit and entry. PSB told me that is not necessary as long as I don't do anything wrong..No one at my office did it before...

This is the case for Guiyang. If it is same for all PSB in China, then you don't need to register again as long as you have same address and same job.

The registration is included in the resident permit application.  If you haven't moved, no problem, as far as I can tell.
