It has to be done in China at specific hospitals certified for this purpose.  A normal health check will consist of blood, urine and an ultrasound.  The whole thing takes less than an hour to do.  But if you have any serious diseases, then you can pretty much count on having your work permit revoked.

Indeed re Pogger, some provinces will accept an overseas health check if it is done and completed on the approved Chinese government medical form with all of the accouterments attached (x-ray, EKG, liver scan, etc., etc.).

Expat John is incorrect here -- any serious disease will not automatically result in getting your work permit cancelled.  AIDS, yes, some STD's yes, other illnesses that can be treated will result in your being denied at that time the medical report; you will be remanded to the No. 1 Hospital in the area for treatment and when the physician at the No. 1 has declared you either treated or in a state of treatment, you will return to the local quarantine station and do the process again with the report of the examining physician of the No 1 hospital in hand.

No, Pogger, I have no personal experience there but I know of an Australian who became quite ill just before his medical with a lung infection, a bacterial one at that.  They held the medical, remanded him to the No. 1 Hospital for treatment, told him to come back when the hospital had deemed him recovered and then they tested him again and with the hospital's paper in hand, gave the clean medical.

The health check to get a visa to come to china isn't a big deal.  But depending on the company you join you may get an annual health check - that often includes anal probing.  Not joking.
