I am in the US and have been offered a job in China. Apparently for the foreign expert visa I need a document from a Dr. in my home country saying I am in good health. I am wondering if there is a specific Chinese health check document the agent should email me to have the Dr. fill out, or if I should just have the Dr. prepare something. I have asked the agent who set up the job but their answer was not clear with the language barrier. Any help is appreciated.

my school sent a document that i had filled out at a hospital in america but when i got to china, i had to do a new physical in china anyway.
Ask for the Medical Exam form at Chinese Embassy in USA.
If you apply for Z visa in Hong Kong, HK Chinese Embassy has proper forms.
I did MExam in HK Hospital in 2012 (or 2011), and I picked the cheapest MExam.
When I present it to the Embassy, they handed me Embassy's ME form, which must be full filed, stamped and signed by examining MD, for the cost of $HK 400. If you come to the Hospital with Medical form, MD knows which tests are required.
New ME is required after you arrive in China for some 350 Rmb or so, usually on the School's budget.

It used to be on HK Embassy website too, but they remove it. It's handy to have form, when you come to the hospice.
