I've done Europe-N.A. calls and video chats and other long distance calls, but it's never been as bad as here.
Here the image quality is usually horrendous, there's a time lag between words or the sound is so bad you can only receive 30-50% of what's said.

Sounds like you have horrible internet service. I've skyped to and from China for years now. I do get a lot of dropped calls (which can be a headache) but most of the time the lag and picture and sound quality arn't bad at all (can be worse if it's the time of day when the entire apartment building seems to be online).
I can not give you an official explanation, but this is what I think it happens.
There are so many persons logged in and using Internet (more so during evenings) that providers are operating at almost capacity of their present equipment, thus speed of transmission is slower than normal. I came to that conclusion based in two facts:
1.- If I try Skype in early morning hours it works fine for me. After midday it slows down considerably, and during evenings is worse than a turtle in reverse.
2.- During evening hours, sometimes I get knock of the Internet by my provider for some periods of time. I can not connect back, and if I do, I will have no Internet acccess. After a while, I can connect back in. I think they do disconnect at random for maybe 30 minutes to an hour to prevent overloading and crashing their system, rather than investing money to expand it.
