Just as the question line states. I'm applying for my visa and there is a supplement form but I'm not sure if it's still necessary after the recent changes to the visa requirements. Does anyone know?

For my recent 'Z' visa application, i had to fill out 2 forms: the main Visa application form, and the supplementary application form.

The second supplementary application form that Sorrel stated might be the same that I had to fill out, A sworn signature by you that you are not lying about any of your past background or current information and that you understand and have read all about the new laws.

That is similar to the Form 'B', as it was called when I applied: just more information regarding your education and Intended work/study on arrival in China, for 'Z' and 'X' applicants. Also if someone else was traveling on your passport, or if you were applying for the visa in a country other than your home country.
Hi Leahkot.The recent visa policy does not have much change about Z visa application. Just as Sorrel said, you have to fill the supplementary form. Because Z visa is for working in China, information like education, personal expertise are normally needed to be provided.
