Say you are at a restaurant. You are using a shared set of chopsticks to grab food with from the spinning table. You are having problems with getting a noodle to your bowl as they are very long. The charming women sitting next to you jumps in and helps. She uses the thick end of her chopsticks to help you tear off the noodle so you can get some to your bowl. .... The thick end of the chopsticks as something that should be acceptably clean ? I've also seen people using the opposite end of the sticks when packing leftover foods into containers.
How is this in any way sanitary ?

You're in a restaurant, it's safer to just accept nothing will be sanitary and not worry too much about it.
Technically, top part of the choppies should be 'cleaner' than bottom, because it's out of the hand-touch, while you're using it.
However, chopsticks shouldn't be considered 'clean', after they're in one's hand-s.
Overall....., English word 'clean' must have different meaning in China than in West.

depends on whether I think the person has washed hands after visiting the tiolet. I prefer mouth bacteria over ass bacteria anytime
Agree with stiggs. If it really sketches you out, fair enough. But then you shouldn't be going out to dinner in that environment, with those people.
If it's work related...then shit. Cost of doing business I guess.
I'm a stickler about bathrooms personally. I won't willingly go anywhere that doesn't have clean bathrooms and offer TP. Because if they don't do those things they believe you are a scumbag and a thief. And I choose not to give money to people who think that of me.

Simple solution:
Ask for a spoon and fork , before you start eating your meal !
and use chopsticks if it was necessary !
Sanitary? Sanitary?? Hahahahahahahah... Nothing is sanitary.
I hate it when people start handing out chopsticks while holding the eating end. Another example of empty head syndrome.
